Class Participation

This Waiver applies to taxidermy class services provided by Drop Dead Gorgeous Taxidermy (“the Provider”).

When you agree to this Waiver, you do so on your own behalf as a participant in taxidermy programs, workshops, and events.

Services include, but are not limited to: hands-on taxidermy instruction, handling of preserved animal materials, use of sharp tools (such as scalpels, needles, pins), and use of preservation agents, fixatives, and other materials associated with the practice of taxidermy.

Assumption of Risk

I ACKNOWLEDGE that it is my responsibility to ensure that I participate only in those activities for which I feel competent, and that I must at all times follow the instructions of Drop Dead Gorgeous Taxidermy staff in relation to the safety and wellbeing of myself and other participants.

I understand that participation involves inherent risks including, but not limited to:

• Use of sharp tools (scalpels, needles, pins, scissors) that may cause cuts or puncture wounds.

• Handling of preserved animal materials that may carry potential allergens, contaminants, or infection risks.

• Exposure to preservation chemicals, solvents, or fixatives which may cause irritation, allergic reaction, or other health effects.

• Risks associated with improper handling of equipment or materials, including injury to self or others.

I agree to wear appropriate clothing and use all required personal protective equipment (PPE) provided or recommended (e.g. gloves, goggles, aprons, masks).

I understand that Drop Dead Gorgeous Taxidermy staff cannot be held responsible for injuries, illness, or accidents that occur as a result of failure to follow instructions, misuse of equipment, or disregard of safety procedures.

By ticking the box or signing below, I confirm I have read this WAIVER AND UNDERSTAND I ASSUME ALL RISK in relation to services provided by Drop Dead Gorgeous Taxidermy.

Medical Release

In the case of an emergency involving myself, I understand that every effort will be made to contact my nominated emergency contact. However, in the event that I cannot be reached, I hereby authorise Drop Dead Gorgeous Taxidermy staff to call 000 to provide urgent care and further authorise me to receive any medical treatment that is necessary and appropriate.

I release and agree to hold Drop Dead Gorgeous Taxidermy harmless from any and all liability, and I hereby assume all risk and responsibility, waiving any claim or cause of action of any nature whatsoever arising while attending classes, workshops, or events.

I further agree to indemnify and hold harmless Drop Dead Gorgeous Taxidermy against any and all claims which may arise from any injury, illness, or harm sustained while participating.

Participant Agreement

• I AGREE I am not under the influence of alcohol, drugs, or any illicit substances while participating.

• I AGREE to comply with all safety protocols and staff instructions during the class.

• I ACKNOWLEDGE that failure to follow safety directions may result in removal from the class without refund.

• I UNDERSTAND that this Waiver is ongoing and will apply to all future occasions I participate in taxidermy services provided by Drop Dead Gorgeous Taxidermy.

• I ACKNOWLEDGE that this document is contractual and may be relied upon in any proceedings by me, my heirs, executors, and assigns.

• I CONFIRM I am 18 years or over and am legally competent to adhere to this agreement. Students under the age of 18 will require a guardian to oversee this agreement.
